Представить основные механизмы возникновения боли при панкреатите и основные подходы к ведению пациента, у которого в качестве ведущего клинического синдрома выступает боль в животе.Основные положения. Преимущественную роль в возникновении боли играет несколько механизмов: прямое токсическое действие повреждающих факторов (в первую очередь — алкоголя), феномен уклонения ферментов, повреждение паренхимы избытком образующихся свободных радикалов, повышение давления панкреатического секрета в протоках, повреждение нервов (нейропатическая боль) и компрессия соседних органов.Ферментные препараты при хроническом панкреатите могут быть показаны не только в качестве заместительной терапии, но и как дополнение комплексного лечения пациентов с выраженными болями в животе. В качестве дополнительной терапии рекомендуются ингибиторы протонной помпы, спазмолитики или прокинетики. Безусловно, при отсутствии желаемых результатов показано дополнительное применение нестероидных противовоспалительных препаратов и анальгетиков, в отдельных случаях — ненаркотических опиатов или прегабалина. Эффективность анальгетиков может быть повышена за счет дополнительного назначения психотропных препаратов, чаще всего — трициклических антидепрессантов. В тяжелых случаях некупирующаяся боль при хроническом панкреатите может выступать в качестве самостоятельного показания к проведению хирургического вмешательства.Заключение. Успех терапии в каждом конкретном случае зависит от своевременного установления и адекватного лечения осложнений, создания мотивации на отказ от вредных привычек, строгого соблюдения диеты на период обострения, а также от создания функционального покоя поджелудочной железе.</p></abstract><trans-abstract xml:lang="en"><sec><title>The aim of review</title><p>The aim of review. To present basic mechanisms of pain at pancreatitis and determine main management approaches in pain-predominant cases.</p></sec><sec><title>Summary</title><p>Summary. Direct toxic action of damaging factors (first of all — alcohol), pancreatic enzyme deviation phenomenon, parenchymal damage by overflow of free radicals, elevation of intraductal pressure, damage of nerves (neuropathic pain) and compression of adjacent organs play primary role in development of pain. Enzyme supplements at chronic pancreatitis can be indicated not only as replacement treatment, but also as addition to complex treatment of patients with severe abdominal pain. Proton pump inhibitors, spasmolytics or prokinetics are recommended as additional therapy. At absence of desirable results application of nonsteroid anti-inflammatory drugs and analgetics, in resistant pain — non-narcotic opiates or pregabalin are indicated. Efficacy of analgetics can be increased by coadministration of psychotropic agents, most often — tricyclic antidepressants. In severe cases intractable pain at a chronic pancreatitis itself can become indication to surgical intervention.</p></sec><sec><title>Conclusion</title><p>Conclusion.
