Отобрано 40 человек, страдавших алкогольным гепатитом тяжелого течения, из двух медицинских центров. Больные были рандомизированы на две группы. Первую группу составили 20 пациентов (16 мужчин и 4 женщины, средний возраст 46,85±11,89 года). Медиана ежедневного употребления алкоголя — 71 год, 25-й и 75-й процентили — 45,5 и 92,0 года соответственно, длительность употребления — 16,85±13,32 года. Средний балл индекса Maddrey (ИМ) — 58,5±26,8. Во вторую группу вошли также 20 пациентов (11 мужчин и 9 женщин, средний возраст 46,10±10,47 года). Медиана употребления алкоголя в сутки составила 73,8 года, 25-й и 75-й процентили – 64,25 и 80,0 года соответственно, длительность употребления — 16,25±5,18 года. Средний балл ИМ составил 79,6±18,7. Группы были сопоставимы по основным клинико-лабораторным характеристикам.Больные первой группы получали преднизолон 40 мг в сутки перорально, второй группы — комбинацию преднизолона 40 мг в сутки перорально и S-аденозил-L-метионина (адомет) 800 мг в сутки внутривенно капельно в течение 7 дней, затем 400 мг 3 раза в сутки перорально до 28 дня терапии. Критерием ответа на лечение служил индекс Lille.В ходе статистической обработки данных использовались критерии: Уилкоксона, Манна–Уитни, χ2. Анализ выживаемости больных проводился с использованием кривой Каплана–Мейера и оценкой значимости по Breslow и LogRank.Результаты. Ответ на терапию был выше в группе пациентов, получавших преднизолон в сочетании с адометом (95 и 65% соответственно, р=0,018). В этой группе отмечена также тенденция к повышению выживаемости по сравнению с группой, получавшей преднизолон (100 и 90%). По частоте развития нежелательных явлений группы достоверно не отличались (р=0,50). Гепаторенальный синдром достоверно чаще встречался у больных, получавших преднизолон (20 и 0%, р=0,03).Выводы. Назначение пациентам с алкогольным гепатитом тяжелого течения комбинации преднизолона и адомета повышает ответ на лечение кортикостероидами, способствует снижению краткосрочной летальности и по сравнению со стандартной терапией достоверно реже приводит к развитию гепаторенального синдрома.</p></abstract><trans-abstract xml:lang="en"><sec><title>Aim of investigation</title><p>Aim of investigation. To evaluate efficacy and safety of S-adenosyl-L-methionine for treatment of alcohol-induced hepatitis of high severity.</p></sec><sec><title>Material and methods</title><p>Material and methods. Overall 40 patients with severe alcoholic hepatitis from two medical centers were selected for the study. Patients were randomly divided in two groups. The first group included 20 patients (16 men, 4 women, mean age 46,85±11,89 years). Median daily alcohol consumption — 71 years, 25-th and 75-th percentiles — 45,5 and 92,0 years respectively, duration of intake — 16,85±13,32 year. The mean score of Maddrey index (MI) was 58,5±26,8. Groups were comparable by basic clinical and laboratory features. Patients of the first group received prednisolone 40 mg/day orally, patients of the second group received combination of prednisolone 40 mg/day orally and S-adenosyl-L-methionine (Adomet) 800 mg/day as intravenous infusions for 7 days, followed by 400 mg tid orally up to the 28 day of treatment. Lille index was utilized as treatment response criterion. During statistical data analysis Wilcoxon, Manna– Whitney and chi-square criteria were used. The survival analysis was done using Kaplan–Meyer curve with Breslow and LogRank significance estimation.</p></sec><sec><title>Results</title><p>Results. Treatment response was higher in the group of patients receiving prednisolone in combination to Adomet (95 and 65% respectively, р=0,018). In this group the trend to increase survival was marked in comparison to the group receiving prednisolone monotherapy (100 and 90%). The rate of adverse events did not differ (р=0,50) significantly between two groups. Hepatorenal syndrome was significantly more frequent in patients receiving prednisolone (20 and 0%, р=0,03).</p></sec><sec><title>Conclusions</title><p>Conclusions.
