Сравнительное изучение эффективности и безопасности применения препарата «Альгерон» в дозах 1,5 и 2,0 мкг/кг и препарата«ПегИнтрон» в сочетании с рибавирином в рамках комбинированной терапии хронического гепатита С (ХГС) с последующим определением терапевтической дозы Альгерона.Материал и методы. В исследование включено 150 ранее не лечившихся взрослых больных ХГС (все генотипы), рандомизированных в 3 группы. В двух основных группах участники получали Альгерон в дозе 1,5 или 2,0 мкг/кг 1 раз в неделю в комбинации с рибавирином, в группе активного контроля – ПегИнтрон 1,5 мкг/кг 1 раз в неделю в комбинации с рибавирином. В качестве первичной конечной точки эффективности использовали частоту достижения быстрого и раннего вирусологических ответов (БВО и РВО).Результаты. Сравнительный анализ частоты достижения вирусологических ответов после 4-й и 12-й недель терапии, биохимического ответа не выявил статистически достоверных различий между группами. БВО зарегистрирован в 64 и 56% случаев в группах пациентов, получавших соответственно 1,5 и 2,0 мкг/кг Альгерона, в группе ПегИнтрона – в 66% (mITT-анализ, p&gt;0,05). Частота достижения РВО к 12-й неделе в группах Альгерона (независимо от назначенной дозы) составила 94%, в группе сравнения (ПегИнтрон) – 88% (p&gt;0,05). Анализ частоты наличия БВО, РВО в зависимости от генотипа вируса гепатита С (HCV) также не выявил статистических отличий между группами. Профиль безопасности использования Альгерона был аналогичен таковому у ПегИнтрона. Комплексная оценка показателей эффективности и безопасности позволила сделать вывод об оптимальной терапевтической дозе Альгерона, равной 1,5 мкг/кг/нед.Выводы. Результаты исследования свидетельствуют о высокой эффективности и безопасности Альгерона в подавлении репродукции HCV и позволяют рекомендовать использование препарата у ранее не леченных пациентов с ХГС в дозе 1,5 мкг/ кг/нед в течение 24–48 нед в зависимости от генотипа HCV.</p></abstract><trans-abstract xml:lang="en"><sec><title>Aim of the study</title><p>Aim of the study. To compare efficacy and safety of Algeron 1.5 and 2.0 μg/kg with PegIntron in combination with ribavirin in the treatment of chronic hepatitis C and to determine therapeutic dose of Algeron.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. 150 adult treatment-naive patients with HCV (all genotypes)were randomized into 3 groups. In the two main groups the patients received Algeron in the dosage of 1.5 or 2.0 μg/kg/week, in the active control group – PegIntron 1.5 μg/kg/week in combination with ribavirin. Primary efficacy endpoints were rapid and early virologic response (RVR and EVR).</p></sec><sec><title>Results</title><p>Results. Comparative analysis of virologic response rate after 4th and 12th weeks of the therapy and biochemical response did not show any statistically significant differences between the groups. RVR was observed in 64% and 56% of patients receiving Algeron 1.5 and 2.0 μg/kg, respectively, in the PegIntron group – in 66% (mITTanalysis, p&gt;0,05).The frequency of EVR in Algeron groups after 12 weeks of treatment (regardless of a dose – 1.5 or 2.0 μg/kg) was 94%, in the reference group (PegIntron) – 88% (p&gt;0,05). RVR and EVR rate analysis according to HCV genotype also did not showed statistically significant differences between the groups. Safety profiles of Algeron and PegIntron were similar. The complex assessment of the efficacy and safety allowed to make the conclusion about the optimal therapeutic dose of Algeron, equal to 1.5 μg/kg/week.</p></sec><sec><title>Conclusions</title><p>Conclusions.
