Оценку результатов у 122 больных с повторными ЭГДС проводили с учетом причин неэффективности предшествующих исследований.Результаты. Причинами повторных гастроскопий послужили: рецидив кровотечения – у 21 (17,2%) больного, из них в 81% случаев он состоялся в течение первых 3 сут от первичного осмотра, неинформативная первая ЭГДС – у 12 (9,8%), контрольная гастроскопия в динамике – у 89 (73%).Часто рецидив кровотечения в ранние сроки развивался вследствие неэффективного эндоскопического гемостаза при активных кровотечениях или отсутствия профилактики при высоком риске рецидива (Forrest IIA–IIB) во время предыдущей гастроскопии. Контрольные динамические гастроскопии у 55 (62%) пациентов выполнялись в 1-е сутки после первичного исследования, что позволило у 6 больных с высоким риском рецидива кровотечения выявить активное рецидивное кровотечение до его клинической манифестации. У 39 (32%) пациентов в дальнейшем потребовалось выполнение повторных гастроскопий, 17 (14%) больных оперированы в экстренном порядке в связи с неэффективным эндоскопическим гемостазом или высоким риском рецидива кровотечения при повторных ЭГДС. Из общего количества больных с повторными гастропатиями летальный исход наступил у 19 (15,6%), из них у 5 в послеоперационном периоде.Выводы. Основными причинами повторных гастроскопий являются рецидивы кровотечения в ранние сроки после первичного гемостаза и нерезультативные исследования. Рецидивы язвенного кровотечения возникают максимально в первые 3 сут после первичного гемостаза. При клинически значимых рецидивах повторная гастроскопия с выполнением противорецидивных мероприятий обоснована у пациентов с высоким операционным риском по сопутствующей патологии (4–5 балов по шкале ASA). Повторные эндоскопические исследования не оказывают значимого влияния на клинические результаты лечения язвенных кровотечений, частоту рецидивов, операционную активность, летальность.</p></abstract><trans-abstract xml:lang="en"><sec><title>Aim of investigation</title><p>Aim of investigation. To estimate necessity and potentials of dynamic esophagogastroduodenoscopies (EGDS) at successful primary hemostasis in patients with high risk of relapse of bleeding.</p></sec><sec><title>Material and methods</title><p>Material and methods. Retrospective analysis of case records of 332 patients hospitalized for bleeding from chronic stomach and duodenum ulcers was carried out. Evaluation of results in 122 patients after repeated EGDSs was carried out with account to causes of inefficiency of previous investigations.</p></sec><sec><title>Results</title><p>Results. Repeated gastroscopies were caused by: relapse of bleeding – in 21 (17,2%) patient, of them in 81% of cases it developed within first 3 days after primary examination, non-informative initial EGDS – in 12 (9,8%), control gastroscopy in dynamics – in 89 (73%). Frequently the bleeding relapses in early terms educed due to ineffective endoscopic hemostasis at active bleedings or absence of prophylaxis at high risk of relapse (Forrest IIA-IIB) at previous gastroscopy. Control gastroscopies in 55 (62%) patients were performed in 1 day after primary investigation that allowed in 6 patients with high risk of bleeding relapse to reveal active recurrent bleeding before its clinical manifestation. In 39 (32 %) patients further required repeated gastroscopies, 17 (14%) patients were urgently operated in relation to ineffective endoscopic hemostasis or high risk of bleeding relapse at repeated EGDSs. Of all patients 19 died (15,6%), of them 5 – in postoperative period.</p></sec><sec><title>Conclusions</title><p>Conclusions. Principal causes of repeated gastroscopies were relapses of bleeding in early period after primary hemostasis and ineffective investigations. Relapses of ulcerative bleeding develop mostly in the first 3 days after primary hemostasis. At clinically significant relapses repeated gastroscopy with preventive actions is proven in patients with high operational risk on concomitant diseases (4–5 points by ASA scale).
