Со временем в отключенных отделах развивается картина колита неинфекционного характера. Исследование посвящено выявлению степени выраженности морфологических изменений в стенке отключенной кишки в зависимости от сроков формирования колостомы.Материал и методы. Материалом для изучения были ткани, полученные при резекции культи нефункционирующих отделов кишки у 255 больных. Морфологические исследования проводились по общепринятой методике с окраской препаратов гематоксилином и эозином. Использовались принципы стереометрического анализа. Реконструктивные и реконструктивно-восстановительные операции с формированием трансверзоректального или сигморектального анастомозов «конец в конец» с ликвидацией колостомы выполнялись в различные сроки, но в основном через 2–2,5 мес (ранняя хирургическая реабилитация).Результаты. Исследования показали развитие процессов перестройки в слизистой оболочке отключенной кишки по типу атрофии с уменьшением количества слизеобразующих эпителиоцитов. Одновременно на фоне формирующегося иммунодефицита происходит развитие воспалительных процессов, вплоть до возникновения микроабсцессов. Установлена прямая взаимосвязь между степенью выраженности «диверсионного» колита и сроком отключения кишки из кишечного пассажа. При выполнении ранней хирургической реабилитации у 125 больных несостоятельности анастомоза не наблюдалось. Число послеоперационных осложнений составило 7,2% (9 пациентов).Выводы. Умеренные изменения воспалительного характера в отключенной кишке не влияют на процессы заживления межкишечного анастомоза при ранней хирургической реабилитации.</p></abstract><trans-abstract xml:lang="en"><sec><title>Aim of investigation</title><p>Aim of investigation. Radical surgery at complicated forms of colorectal cancer frequently results in colostoma formation. In due course in diverted regions colitis of non-infectious character educes. The study is devoted to revealing of degree of morphological changes of defunctioning loop wall in relation to terms of colostoma formation.</p></sec><sec><title>Material and methods</title><p>Material and methods. Tissues received at resection of nonfunctioning regions of intestinal stump in 255 patients were subject of the study. Morphological investigation was carried out under the practical standard with staining of samples by hematoxylin and eosine. Stereometric analysis principles were used. Reconstructive and restitutional operations with formation of transverse-rectal or sigmoido-rectal anastomoses «end-to-end» with liquidation of colostoma in various terms, but mostly in 2–2,5 months (early surgical rehabilitation).</p></sec><sec><title>Results</title><p>Results. Investigations demonstrated development of processes of remodeling in mucosa of defunctioning loop of atrophic type with decrease of quantity of mucus-producing epithelial cells. Simultaneously on a background of developing immunodeficiency there was progression of inflammatory processes, up to microabscess development. The direct interrelation between degree of «diversive» colitis and terms of intestine diversion from intestinal passage was is found. At early surgical rehabilitation in 125 patients no incompetence of anastomosis was observed. The postoperative morbidity rate was 7,2 % (9 patients).</p></sec><sec><title>Conclusions</title><p>Conclusions.
