Дифференциальный диагноз между хирургическим и коагулопатическим характером кровотечения после проведенного инвазивного вмешательства помогает выбрать оптимальный алгоритм ведения больных. Представленное клиническое наблюдение иллюстрирует тактику обследования и лечения таких пациентов.Основные положения. В клинику госпитализирован пациент К. 54 лет. Поводом для поступления послужило развитие декомпенсированного цирроза печени токсической и вирусной этиологии. По тяжести состояния пациент был направлен в отделение интенсивной терапии. С целью проведения инфузионной терапии возникла необходимость в установке центрального венозного катетера. Катетеризация осложнилась кровотечением из канала катетера. Применение метода тромбоэластографии помогло дифференцировать нарушение системного гемостаза от механического повреждения сосудов при выполнении катетеризации. Был подтвержден коагулопатический характер осложнения. Выполнение тромбоэластографии в динамике позволило оценить эффективность проводимой трансфузионной терапии и наметить пути ее коррекции.Заключение. Оценивая представленное клиническое наблюдение, можно сделать вывод о необходимости проведения тромбоэластографии у больных с кровотечением, которое может происходить как по причине нарушения гемостаза (у пациентов с расстройством белково-синтетической функции печени), так и вследствие инвазивного вмешательства. Данные тромбоэластографии помогут адекватно подобрать гемостатическую терапию и избежать дополнительных исследований, а, возможно, и хирургических вмешательств.</p></abstract><trans-abstract xml:lang="en"><p>The aim of the publication. The control of hemostasis is important investigation in clinical practice. The differential diagnosis between surgical and coagulopathic nature of bleeding after the invasive intervention helps to choose optimal algorithm of patient management. Presented clinical case illustrates approach to investigation and treatment of such patients.</p><sec><title>Original positions</title><p>Original positions. Patient K., male 54 years was hospitalized to the clinic for decompensated liver cirrhosis of toxic and viral etiology. Due to high severity of patient’s state he was directed to intensive care unit. Intravenous infusion therapy required installation of central venous catheter, that was complicated by catheter bleeding. Application of thromboelastography helped to differentiate disorder of systemic hemostasis from mechanical damage of blood vessels at catheterization. Coagulopathic nature of complication has been confirmed. Dynamic control of thromboelastography allowed to estimate efficacy of transfusion therapy and to plan ways of its correction.</p></sec><sec><title>Conclusion</title><p>Conclusion. Estimating the submitted clinical case, it is possible to conclude on necessity of thromboelastography in patients with bleeding, which can develop both due to disorder of hemostasis (at patients with failure of protein-synthetic liver function), and due to invasive intervention.
