Определить клиническое значение уровня вирусной нагрузки (УВН) HBV в крови у пациентов с естественным течением хронической HBV­моноинфекции.Материал и методы. Обследовано 149 пациентов. Среди них 68 (45,6%) женщин и 81 (54,4%) мужчина в возрасте от 16 лет до 71 года. Молекулярно-­биологическое исследование на наличие в крови ДНК вируса гепатита В проводилось трехкратно с интервалом в 6 мес методом полимеразной цепной реакции (ПЦР) в режиме «реального времени» (чув­ствительность метода 103 копий/мл).Результаты. При проведении трехкратного комплексного обследования было выделено 20% неактивных носителей HBsAg. При однократном ПЦР-­исследовании у большинства (59,5%) пациентов с хроническим гепатитом В (ХГВ) ДНК HBV в крови не определялась, в то время как при двукратном и трехкратном исследовании число больных с неопределяемым уровнем ДНК HBV заметно сократилось (41 и 12,5% соответственно). Анализ показал, что у больных ХГВ существует прямая корреляционная зависимость между активностью аланинамино­транс­феразы (АлАТ) и УВН ДНК HBV (r=0,385, р=0,008). Лишь в 9% случаев регистрировался постоянно высокий УВН, преимущественно на фоне повышенной активности АлАТ. У 21,7% пациентов с естественным течением хронической HBV-­моноинфекции была диагностирована прогрессирующая форма ХГВ, в связи с чем они были признаны кандидатами для назначения противовирусной терапии.Выводы. В проведенном исследовании выявлена прямая корреляционная зависимость между показателями УВН HBV и активностью АлАТ у пациентов с естественным течением хронической HBV-­моноинфекции. Только комплексное динамическое обследование таких больных поможет выделить абсолютных кандидатов для назначения противовирусной терапии и определить долю неактивных носителей HBsAg.</p></abstract><trans-abstract xml:lang="en"><sec><title>Aim of investigation</title><p>Aim of investigation. To determine clinical value of viral load level (VLL) of HBV in blood at patients with natural course of chronic HBV-monoinfection.</p></sec><sec><title>Material and methods</title><p>Material and methods. Overall 149 patients were investigated. Of them 68 (45,6%) were women and 81 man (54,4%) in the age of 16 to 71 years. Molecularbiological analysis for the presence of hepatitis B virus DNA in blood was carried out three times with 6 months intervals by polymerase chain reaction (PCR) method in a «real time» mode (test-sensitivity 103 copies/ml).</p></sec><sec><title>Results</title><p>Results. At triple complex investigation 20% of HBsAg inactive carriers have been selected. At a single PCR – test at the majority (59,5%) of patients with chronic hepatitis B (CHB) no HBV DNA was determined in a blood, while at two-times and triple study the number of patients with not determined HBV DNA level reduced apparently (41 and 12,5% respectively). Analysis has indicated, that CHB patients have direct correlation between alanine transaminases (ALT) activity and HBV DNA VLL (r=0,385, р=0,008). Only in 9% of cases constantly high VLL was registered, mainly, on a background of elevated activity of ALT. The progressing form of CHB has been diagnosed in 21,7% of patients with natural course of chronic HBV-monoinfection for this they have been recognized as candidates for antiviral therapy.</p></sec><sec><title>Conclusions</title><p>Conclusions. In original study the direct correlation between parameters of HBV VLL and ALT activity at patients with natural course of chronic HBV-monoinfection was revealed.
