Показать современные возможности применения препаратов висмута с целью повышения эффективности эрадикации инфекции Helicobacter pylori (H. pylori). Основные положения. В свете профилактики язвенной болезни и рака желудка борьба с инфекцией H. pylori приобретает все большее значение. Вместе с тем эффективность ряда схем эрадикации, в первую очередь стандартной тройной терапии, неуклонно снижается из-за растущей устойчивость штаммов H. pylori к антибиотикам, прежде всего к кларитромицину. Все более широкое распространение получает точка зрения, согласно которой необходимо отказаться от эмпирического назначения стандартной тройной терапии и предварительно определять устойчивость штаммов H. pylori к кларитромицину с помощью получения и последующего исследования культуры микроорганизмов или проведения полимеразной цепной реакции в режиме реального времени. Альтернативой стандартной тройной терапии служит квадротерапия с препаратами висмута, основным из которых в настоящее время является висмута трикалия дицитрат (ВТД). Показана возможность комбинации ВТД не только с тетрациклином, но и с другими антибиотиками. Добавление ВТД к тройным схемам, содержащим кларитромицин, левофлоксацин, метронидазол, позволяет на 30-40% повысить эффективность эрадикации при резистентности H. pylori к данным антибиотикам. Важными преимуществами ВТД служат отсутствие формирования резистентности к препарату при его применении, сочетание бактерицидного и цитопротективного эффектов, хорошая переносимость. Заключение. Добавление ВТД к тройным схемам эрадикации, содержащим различные антибиотики (кларитромицин, левофлоксацин, метронидазол), дает возможность существенно повысить эффективность эрадикации даже при наличии резистентности H. pylori к указанным антибиотикам.</p></abstract><trans-abstract xml:lang="en"><p>Aim of review. To demonstrate modern potential of bismuth agents administration for increasing of Helicobacter pylori (H. pylori) eradication efficacy. Summary. Prevention of peptic ulcer and stomach cancer makes eradication of H. pylori infection more and more important. At the same time efficacy of some eradication modes, first of all - that of standard triple therapy, steadily decreases because of growing antibiotic resistance of H. pylori strains, first of all - clarithromycin resistance. The concept that empirical prescription of standard triple therapy should be abandoned in favor of initial H. pylori clarithromycin-resistance testing by culture or real-time polymerase chain reaction became widely accepted. Bismuth-based quadrotherapy mainly - with bismuth tripotassium dicitrate (BTD) is alternative to standard triple mode therapy nowadays. Potential of BTD combination not only to tetracycline but to other antibiotics as well was demonstrated. Addition of BTD to triple therapy containing clarithromycin, levofloxacin, metronidazole allows to increase eradication efficacy for 30-40% at H. pylori-resistant strains. Absence of H. pylori resistance development to BTD, combined antibacterial and cytoprotective effects as well as good tolerance are important advantages of the drug.  Conclusion.
