Цель:  оценить эффективность трансплантации печени в лечении нерезектабельной ГЦК, развившейся в отсутствие фонового заболевания печени.

Материал и методы

Материал и методы. 6 пациентам с нерезектабельной ГЦК выполнена ОТП. Отдаленные результаты сравнивали с пациентами, перенесшими резекции печени по поводу ГЦК на поздней стадии.

Результаты

Результаты.  Гепатоцеллюлярная карцинома (ГЦК) — одна из наиболее распространенных форм рака, встречающаяся в подавляющем большинстве случаев у пациентов с циррозом печени и вирусным гепатитом. Лишь в 10 % наблюдений ГЦР развивается в нецирротической печени, как правило, у молодых и соматически сохранных пациентов. 1-, 3-, 5-летняя безрецидивная и общая выживаемость в группе трансплантированных больных были достоверно выше, чем в контрольной группе.

Вывод

Вывод.  Трансплантация печени показана пациентам с нерезектабельной ГЦК в отсутствие предсуществующего цирроза печени и внепеченочного распространения. Большой размер опухоли и макрососудистая инвазия не должны являться противопоказанием к ОТП у таких пациентов. 

Aim

Aim.  The aim of the study is to determine the effectiveness of liver transplantation (LT) in the treatment of unresectable hepatocellular carcinoma (HCC) occurred in normal liver.

Material and methods

Material and methods.  6 patients with unresectable HCC underwent orthotopic liver transplantation (OLT). The long-term OLT results were compared with survival results of liver resection in patients with late stage HCC.

Results

Results.  Hepatocellular carcinoma is one of the most common types of cancer, which occurs mainly in patients with liver cirrhosis and chronic viral hepatitis. Only about 10 % of HCC develops in non-cirrhotic liver among young and somatically healthy patients. 1-, 3-, 5-year recurrence-free and overall survival in LT group was significantly better than in the control group.

Conclusion

Conclusion.  LT is indicated for patients with unresectable HCC in non-cirrhotic liver and its extrahepatic localization. A large tumor size and macrovascular invasion should not be a contraindication for LT in such patients.
