Эозинофильный эзофагит (ЭоЭ) — это хроническое иммуноопосредованное заболевание пищевода, характеризующееся симптомами эзофагеальной дисфункции и выраженной эозинофильной инфильтрацией слизистой оболочки органа. Диагностика ЭоЭ основывается на данных клинической картины заболевания (дисфагия, эпизоды вклинения пищи в пищевод, боль в грудной клетке без связи с глотанием), а также на совокупности эндоскопических и гистологических признаков. Критерием установления диагноза служит эозинофильная инфильтрация слизистой оболочки пищевода с плотностью эозинофилов ≥ 15 в поле зрения микроскопа при большом увеличении (×400) по крайней мере в одном из биоптатов (около 60 эозинофилов в мм2). Исследование общего IgE, эозинофилия периферической крови, результаты кожных аллергопроб имеют вспомогательное значение для диагноза. Для лечения ЭоЭ используется несколько подходов, включающих применение ингибиторов протонной помпы (ИПП), топических глюкокортикостероидов (ГКС), а также элиминационные диеты. Выбор терапии должен быть индивидуализированным, с обязательной оценкой эффективности инициального курса через 6–12 недель при эзофагогастродуоденоскопии с забором биоптатов. Эндоскопическая дилатация должна рассматриваться у пациентов, страдающих выраженной дисфагией на фоне сужения пищевода.</p></sec><sec><title>Заключение</title><p>Заключение. Рост заболеваемости ЭоЭ в сочетании с преимущественным поражением детей и лиц молодого возраста, хроническое течение заболевания с необходимостью длительной поддерживающей терапии делают ЭоЭ значимой проблемой гастроэнтерологической практики.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. This paper presents guidelines on the diagnostics and treatment of eosinophilic esophagitis, which can be used by practitioners in their everyday practice.</p></sec><sec><title>Summary</title><p>Summary. Eosinophilic esophagitis (EoE) is a chronic immune-mediated disease of the esophagus characterized by the symptoms of esophageal dysfunction and a pronounced eosinophilic infiltration of the esophageal mucosa. The EoE diagnostics is based on the clinical manifestations of the disease (dysphagia, food impaction, chest pain regardless of swallowing), as well as on the combination of endoscopic and histological signs. The diagnostic criterion is the eosinophilic infiltration of the esophageal mucosa with an eosinophil density of ≥ 15 per high power field (×400) in at least one of the biopsy specimens (about 60 eosinophils in 1 mm2). Total IgE levels, peripheral blood eosinophilia and skin allergy tests are considered to be additional diagnostic means. Several approaches are used for the treatment of EoE, including proton pump inhibitors (PPIs) and topical glucocorticosteroids (GCS), as well as elimination diets. The choice of therapy should be individualized, with the mandatory assessment of the treatment efficacy after 6–12 weeks using esophagogastroduodenoscopy with biopsy sampling. Endoscopic dilatation should be considered in patients suffering from severe dysphagia due to esophagus stricture.</p></sec><sec><title>Conclusion</title><p>Conclusion.
