Изучить вклад билиарного сладжа (БС) в развитие хронического панкреатита (ХП) на основании исследования пациентов со сладжем желчного пузыря и достоверными признаками ХП. Оценить клиническую эффективность и безопасность терапии гимекромоном по клиническим, лабораторным признакам, а также динамике размеров и сократимости желчного пузыря.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В открытое одноцентровое клиническое исследование были включены 30 пациентов с ХП, который был верифицирован согласно Кембриджским критериям. Всем пациентам был назначен гимекромон (препарат Одестон®) в дозе по 400 мг 3 раза в сутки, в течение 3 недель. Анализ эффективности и безопасности исследуемого препарата проводили на 21-й день лечения на основании результатов лабораторных исследований, данных ультразвукового (УЗ) исследования органов брюшной полости, данных холецистографии и эндосонографии панкреатобилиарной зоны, оценки качества жизни больных по данным опросника SF-36, регистрации частоты и выраженности нежелательных явлений (НЯ).</p></sec><sec><title>Результаты</title><p>Результаты. Признаки ХП обнаружены у 6,3 % пациентов с БС желчного пузыря. ХП достоверно более часто наблюдался при замазкообразной желчи (33,3 %, χ2 = 38,21, p &lt; 0,00001). Площадь большого дуоденального сосочка (БДС) была ниже нормы у 78 % больных. Согласно факторному анализу на фоне монотерапии гимекромоном наблюдалось уменьшение абдоминальной боли, тошноты, тяжести в животе и вздутия живота. К концу курса терапии достоверно повысилось качество жизни по шкале соматической боли «BP» опросника SF-36.</p></sec><sec><title>Выводы</title><p>Выводы. Установлено, что билиарный сладж (в том числе недиагностированный при рутинном обследовании) может быть фактором развития хронического панкреатита. Медикаментозная коррекция билиарных расстройств при хроническом панкреатите должна включать в себя назначение селективных спазмолитиков. Гимекромон имеет хорошую переносимость и безопасность, в короткий срок нормализует моторную функцию билиарного тракта, тонус сфинктеров и купирует симптоматику у больных с ХП.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the contribution of biliary sludge (BS) to the development of chronic pancreatitis in patients with gallbladder sludge and verified chronic pancreatitis; to evaluate the clinical efficacy and safety of hymecromone therapy according to clinical and laboratory signs, as well as dynamics of gallbladder size and contractility.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. An open single-centre clinical trial included 30 patients with chronic pancreatitis (CP), which was diagnosed according to the Cambridge criteria. All patients received hymocromone (Odeston®) 400 mg tid for 3 weeks. An analysis of the efficacy and safety of the studied drug was performed on the 21st day of treatment based on the results of laboratory tests, abdominal ultrasound, cholecystography and endosonography of the pancreatobiliary zone, quality-of-life assessment according to the SF-36 questionnaire, the frequency and severity of adverse effects (AE).</p></sec><sec><title>Results</title><p>Results. CP signs were found in 6.3 % of patients with gallbladder BS. CP was significantly more frequent in patients with putty-like bile (33.3 %, χ2 = 38.21, p &lt; 0.00001). The area of the major duodenal papilla (MDP) was below normal in 78% of patients. According to factor analysis, monotherapy with hymocromone resulted in a decrease in abdominal pain, nausea, heaviness in the abdomen and bloating. By the end of the therapy, the quality of life according to the “BP” bodily pain scale of the SF-36 questionnaire significantly increased.</p></sec><sec><title>Conclusions</title><p>Conclusions. Biliary sludge (including undiagnosed forms during routine examination) was found to be a factor in the development of CP. The medical correction of biliary disorders in CP should include selective antispasmodics.
