Настоящие рекомендации Российской гастроэнтерологической ассоциации по клиническому применению манометрии высокого разрешения при заболеваниях пищевода разработаны с целью обоснования показаний к проведению исследования, унификации терминологии и интерпретации полученных данных клиницистами.</p></sec><sec><title>Основные положения</title><p>Основные положения. В 2018 году на совместном заседании Российской гастроэнтерологической ассоциации и Российской группы нейрогастроэнтерологии и моторики были утверждены единая терминология и классификация нарушений двигательной функции пищевода, диагностируемых методом манометрии высокого разрешения.</p><p>Пациенты гастроэнтерологического профиля часто предъявляют жалобы, характерные для заболеваний пищевода, среди которых можно выделить дисфагию, регургитацию, изжогу, боль в грудной клетке, отрыжку. Для исключения эрозивно-язвенного поражения, эозинофильного эзофагита, органических изменений таким пациентам должна быть выполнена эзофагогастродуоденоскопия, биопсия. После исключения повреждений слизистой оболочки и обструкции просвета пищевода как причин возникновения симптомов рекомендуется проведение манометрии высокого разрешения. Этот метод исследования пищевода признан «золотым стандартом» в диагностике нарушений его двигательной функции.</p><p>Метод манометрии высокого разрешения дает возможность детально исследовать интегральные количественные и качественные характеристики двигательной функции пищевода и их специфические нарушения, анализировать процесс распространения сокращения по пищеводу и строго скоординированную синхронную перистальтику мышц верхнего пищеводного сфинктера, грудного отдела и нижнего пищеводного сфинктера, нарушение которой может приводить к развитию ахалазии кардии, эзофагоспазма, грыжи пищеводного отверстия диафрагмы, неэффективной перистальтики и другим нарушениям двигательной функции.</p></sec><sec><title>Заключение</title><p>Заключение. Манометрия высокого разрешения — это относительно новый метод исследования двигательной функции пищевода, который завоевывает все большую популярность в клинической практике. Он позволяет врачу получить сведения, которые могут существенным образом изменить тактику ведения пациента и дифференцированно подходить к выбору назначений. Настоящие рекомендации обобщают всю имеющуюся актуальную информацию, однако по мере накопления нового клинического материала и систематизации данных с позиции доказательной медицины они будут обновляться, чтобы обеспечить российских гастроэнтерологов наиболее значимыми и практически-ориентированными материалами.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. Current recommendations of the Russian Gastroenterological Association on clinical use of high-resolution manometry in diagnosis of esophageal disorders are intended to assist in clinical decision making, terminology standardisation and interpretation of clinical data.</p></sec><sec><title>Key points</title><p>Key points. In 2018, a joint meeting of the Russian Gastroenterological Association and Russian Neurogastroenterology and Motility Group approved unified terminology and classification of esophageal motor function disorders for high-resolution manometry diagnosis.</p><p>Gastrointestinal patient complaints typically concern esophageal disorders such as dysphagia, regurgitation, heart-burn, chest pain or belching. To exclude erosive and ulcerative lesions, eosinophilic esophagitis and organic changes, esophagogastroduodenoscopy and biopsy are recommended in pre-treatment. Upon excluding mucosal lesions and esophageal lumen obstruction as causal for symptoms, use of high-resolution manometry is recommended. This method of esophageal examination has become the “gold standard” in diagnosis of motor disorders.</p><p>High-resolution manometry enables detailed investigation of integral quantitative and qualitative characteristics of esophagus motor function and specific related disorders, analysis of esophageal contractile propagation and strictly coordinated synchronous peristalsis of upper esophageal sphincter, esophagus and lower esophageal sphincter, which malfunction may provoke development of achalasia, esophagospasm, hiatal hernia, ineffective eso pha geal motility and other motor disorders.</p></sec><sec><title>Conclusion</title><p>Conclusion. High-resolution manometry is a relatively new method for study of esophagus motor function gaining increasingly wide application in clinical practice. It enables a medical professional to obtain evidence that may critically affect the choice of optimal patient care strategy and effective treatment.
