Микофенолата мофетил (ММФ) является эффективным иммунодепрессивным средством, к его побочным эффектам относится воздействие на желудочно-кишечный тракт (ЖКТ). Пациент 37 лет, страдающий сахарным диабетом 1-го типа, поступил в отделение гастроэнтерологии с клиникой диареи с гематохезией. В 2012 г. произведена додиализная трансплантация почки от родственного донора в связи с диабетической нефропатией, с этого момента до настоящего времени постоянно принимал ММФ в дозе 2000 мг в сутки. При диагностической колоноскопии выявлен катарально-язвенный колит, в анализе кала — токсины C. difficile. Проведенные исследования позволили исключить язвенный колит, ишемический колит, болезнь «трансплантат против хозяина». При отмене ММФ и назначении ванкомицина отмечена положительная клинико-эндоскопическая динамика.Заключение. Приведенное наблюдение демонстрирует клиническую картину и алгоритм диагностики колита, ассоциированного с приемом ММФ. Особенностью данного случая служит сочетание с инфекцией C. difficile</p></abstract><trans-abstract xml:lang="en"><p>Aim. A clinical observation of colitis conditioned by mycophenolate mofetil intake and concomitant Clostridium difficile-associated disease.Key points. Mycophenolate mofetil (MMF) is an active immunosuppressant with side effects affecting gastrointestinal tract (GIT). A 37-yo male patient with type 1 diabetes mellitus was admitted at a gastroenterology unit with clinical signs of diarrhoea with haematochezia. A history of diabetic nephropathy and related-donor pre-dialysis kidney transplantation in 2012, since when MMF intake was 2000 mg daily. Catarrhal ulcerative colitis in colonoscopy, C. difficile toxins in pathogen stool panel. Ulcerative, ischaemic colitises and the graft-versus-host disease were ruled out in examination. A positive clinical and endoscopic trend was observed upon MMF withdrawn and start of vancomycin.Conclusion. The case presented illustrates the clinical picture and diagnostic algorithm in MMF-associated colitis.
