Воспалительные заболевания кишечника и статины

Аннотация
Цель обзора. Представить современные данные о возможностях применения статинов у пациентов с воспалительными заболеваниями кишечника (ВЗК).
Основные положения. Статины обладают иммуномодулирующими эффектами, изучение которых в лечении ВЗК представляет интерес. Приведены результаты экспериментальных исследований, в которых были продемонстрированы положительные эффекты статинов на различные характеристики течения ВЗК. Применение статинов приводило к снижению гистологической активности на моделях с DSS- и TNBS-индуцированными колитами. Показано снижение эпителиальной проницаемости и уменьшение крипт на фоне применения правастатина. Установлено изменение состава кишечной микрофлоры под действием симвастатина, имеющее потенциальное противовоспалительное действие. Представлены результаты клинических исследований, в которых назначение аторвастатина больным с ВЗК приводило к статистически значимому снижению сывороточных маркеров воспаления и индексу активности язвенного колита. Ретроспективные клинические исследования показали различное влияние статинов на риск возникновения ВЗК.
Заключение. В настоящее время установлен ряд потенциально протективных эффектов статинов в отношении воспалительных заболеваний кишечника. Необходимо проведение хорошо планированных рандомизированных клинических исследований, которые позволят оценить эффекты статинов при ВЗК.
