Хирургические методы лечения экстрасфинктерных и транссфинктерных свищей прямой кишки

Цель обзора. Ознакомить с данными литературы по хирургическому лечению экстрасфинктерных и транссфинктерных свищей прямой кишки.
Основные положения. Лечение свищей прямой кишки остается сложной задачей из-за опасности частого возникновения рецидива заболевания — от 4,7 до 60%. Радикальные методы лечения излишне травматичны и приводят к формированию выраженных рубцовых изменений в структурах анального канала, что сопровождается высоким риском развития недостаточности анального сфинктера — до 83%. В настоящее время в литературе встречается все больше методов, в которых с целью устранения дефекта ткани и улучшения регенерации с высокой эффективностью используются различные виды имплантов. Применение коллагеновых имплантов позволяет снизить травматизацию волокон сфинктера, тем самым сводить к минимуму развитие недостаточности последнего.
Заключение. Исключение развития послеоперационной анальной инконтиненции является приоритетной задачей в лечении экстрасфинктерных свищей прямой кишки.
