Противовирусная терапия и регресс фиброза печени при хроническом гепатите В
Аннотация
Цель обзора. Изучение влияния противовирусной терапии на регресс фиброза при хроническом гепатите В.
Основные положения. В статье представлен анализ наиболее крупных исследований, оценивавших влияние терапии интерферономa и аналогами нуклеозидов/нуклеотидов на динамику фиброза печени. Уменьшение выраженности фиброза (с целью профилактики развития цирроза печени и гепатоцеллюлярной карциномы) является одним из основных критериев эффективности лечения хронических заболеваний печени. В настоящее время обсуждаются различные стратегии воздействия на фиброз, которые включают устранение этиологического фактора, подавление воспаления и специфическую антифибротическую терапию.
Заключение. Успешная противовирусная терапия при хроническом гепатите В (стойкое подавление репликации вируса и активности воспаления в печени) приводит к регрессу фиброза, в том числе на стадии цирроза, и ассоциируется с улучшением выживаемости больных.
